Care management activities through the AAA serve as a coordinative link between the identification of consumer needs and the timely provision of appropriate services to meet consumer’s needs. Those consumers receiving OPTIONS services will be care managed, along with those receiving Protective Services and Dom Care Services. Consumers can receive care management as a standalone service, these would be consumers in need of the support and assistance of a care manager.
